Jack Donald Patchak's Obituary
Jack D Patchak, 81, passed away Tuesday, August 4, 2020 at home surrounded by his loving family. He was born to Albert and Helen (Ciolkosz) of Cadott, Wi., April 21, 1939.
Jack married the love of his life Loraine (“Jitter” Polzin) on August 22, 1959. They have 2 sons, Dennis (Renee) of Cave Creek, Az. and Mark (Sue) of Van Dyne, Wi. Four grandchildren, Ryan (Katherine) and Amber of Phoenix, Alex and Austin of Van Dyne. Two great-grandchildren Jack and Nora of Phoenix.
Jack was preceded in death by his parents, brothers George and Raymond, sisters, Susan (Cook) and Mae (Joswiak). He is survived by brothers, Michael and Richard, sisters Charlotte (Pyzyna), Ceil (Mikulski) and Joann (Zimmerman).
Jack was an avid Cubs and Packer fan, loved playing cards and fishing. Enjoyed multiple trips with his sons fishing in Canada. Family was top priority for Jack followed by his love of sports. He spent many years coaching youth and teaching his sons about life lessons.
Private memorial services were held for Jack at the Poklasny Funeral Home.
In Lieu of flowers a memorial fund will be established.
